RESUMO

The year 2020 is an historic year on many accounts. It was to be an international year of celebration for nurses and midwives but turned into a year of turmoil with the emergence of the coronavirus pandemic and a call for justice and social awareness. Nurses were in the forefront and demonstrated courage and heroic efforts in the face of devastating events. Advanced practice nurses need to document their stories. This becomes our legacy. In an effort to preserve our legacy, the American Association of Nurse Practitioners history committee is in the process of identifying the existing historical documents for preservation.

RESUMEN Introducción: La aparición del término enfermería de práctica avanzada es relativamente reciente. En Cuba, las formas de educación en este tipo de práctica han sido evidentes desde los primeros años posteriores al triunfo de la revolución hasta la actualidad. Objetivo: Sistematizar la definición e historia de la enfermería de práctica avanzada. Métodos: Se realizó una revisión bibliográfica sistemática para desarrollar un análisis crítico reflexivo del contenido de artículos originales y de revisión, publicados en español e inglés entre 1999 y 2015. Las palabras clave utilizadas fueron "enfermería de práctica avanzada", "definición de enfermería de práctica avanzada", "historia de enfermería de práctica avanzada" la búsqueda fue realizada en las bases de datos SciELO, Ebsco, LILACS y MEDLINE/Pubmed. Conclusiones: La revisión de la historia de la enfermería de práctica avanzada evidencia que es un proceso en constante movimiento que incluye la superación, capacitación, educación continuada y permanente, la formación académica y surge para responder a las demandas sociales de la población, se consolida gracias al desarrollo de diferentes estructuras organizacionales, educacionales y de investigación(AU)


ABSTRACT Introduction: The appearance of the term advanced nursing practice is relatively recent. In Cuba, the forms of education in this practice have been apparent since the first years after the triumph of the revolution until today. Objective: To systematize the definition and history of advanced practice nursing. Methods: A systematic literature review was conducted to develop a thoughtful critical analysis of original content and review articles published in Spanish and English between 1999 and 2015. The keywords used were "advanced nursing practice", "definition of nursing advanced practice "," history of advanced nursing practice "search was conducted on the basis of SciELO, Ebsco, LILACS and MEDLINE / PubMed data. Conclusions: The review of the history of advanced practice nursing evidence is a process in constant movement that includes overcoming, training, continuous and continuing education, academic training and comes to meet the social demands of the population, consolidated through the development of different organizational, educational and research structures(AU)

In 1952, Hildegard Peplau published her textbook Interpersonal Relations in Nursing: A Conceptual Frame of Reference for Psychodynamic Nursing. This was the same year the American Psychiatric Association (APA) published the first edition of the Diagnostic and Statistical Manual of Mental Disorders (1st ed.; DSM-I; APA). These events occurred in the context of a rapidly changing policy and practice environment in the United States after World War II, where the passing of the National Mental Health Act in 1946 released vast amounts of funding for the establishment of the National Institute of Mental Health and the development of advanced educational programs for the mental health professions including nursing. This article explores the work of two nurse leaders, Hildegard Peplau and Dorothy Mereness, as they developed their respective graduate psychiatric nursing programs and sought to create new knowledge for psychiatric nursing that would facilitate the development of advanced nursing practice. Both nurses had strong ideas about what they felt this practice should look like and developed distinct and particular approaches to their respective programs. This reflected a common belief that it was only through nurse-led education that psychiatric nursing could shape its own practice and control its own future. At the same time, there are similarities in the thinking of Peplau and Mereness that demonstrate the link between the specific social context of mental health immediately after World War II and the development of modern psychiatric nursing. Psychiatric nurses were able to gain significant control of their own education and practice after the war, but this was not without a struggle and some limitations, which continue to impact on the profession today.

RESUMO

PURPOSE: This paper celebrates the 60th anniversary of the Western Institute of Nursing, the nursing organization representing 13 states in the Western United States, and envisions a preferred future for nursing practice, research, and education. BACKGROUND: Three landmark calls to action contribute to transforming nursing and healthcare: the Patient Protection and Affordable Care Act of 2010; the Institute of Medicine report Future of Nursing: Leading Change, Advancing Health; and the report Advancing Healthcare Transformation: A New Era for Academic Nursing. Challenges abound: U.S. healthcare remains expensive, with poorer outcomes than other developed countries; costs of higher education are high; our profession does not reflect the diversity of the population; and health disparities persist. Pressing health issues, such as increases in chronic disease and mental health conditions and substance abuse, coupled with aging of the population, pose new priorities for nursing and healthcare. DISCUSSION: Changes are needed in practice, research, and education. In practice, innovative, cocreated, evidence-based models of care can open new roles for registered nurses and advanced practice registered nurses who have knowledge, leadership, and team skills to improve quality and address system change. In research, data can provide a foundation for clinical practice and expand our knowledge base in symptom science, wellness, self-management, and end-of-life/palliative care, as well as behavioral health, to demonstrate the value of nursing care and reduce health disparities. In education, personalized, integrative, and technology-enabled teaching and learning can lead to creative and critical thinking/decision-making, ethical and culturally inclusive foundations for practice, ensure team and communication skills, quality and system improvements, and lifelong learning. CONCLUSION: The role of the Western Institute of Nursing is more relevant than ever as we collectively advance nursing, health, and healthcare through education, clinical practice, and research.

RESUMO

The 2010 Institute of Medicine report, the Future of Nursing, recommended that nurses work to the "full extent of their training" to address the primary healthcare needs of United States citizens. This article identifies and describes historical antecedents, cornerstone documents, and legislative acts that served to set the stage for today, laying the groundwork for an expanded role for advanced practice nurses in the 21st century. Beginning with Lillian Wald's work in Henry Street Settlement in 1893, through Mary Breckenridge's founding of the Frontier Nursing Service in 1925, the discussion describes how nurses provided access to care for thousands of urban and rural citizens throughout the United States in the past. The article also discusses political forces at midcentury and the creation of the nurse practitioner role with the premise that nurses can learn from these early initiatives to create new models for nurses' roles in primary care today.

Introducción: cambios sociales y políticos han provocado un aumento de la demanda de los servicios de salud favoreciendo la aparición de distintos perfiles profesionales bajo el término de Enfermera de Práctica Avanzada (EPA). La EPA es una enfermera que ha adquirido una base de conocimiento experto, habilidades para la toma de decisiones y competencias clínicas para una práctica ampliada. En Españ;a, debido al proceso de Bolonia y los cambios en la Universidad, como la formación de posgrado, han empezado a emerger estos perfiles. Objetivo: describir la experiencia en la implementación del rol avanzado en enfermería en una unidad de hospitalización de oncología. Desarrollo: a través de las nueve etapas del marco conocido como Proceso Participativo para la Enfermería de Práctica Avanzada centrado en el Paciente y basado en la Evidencia (PEPPA) se explica el proceso de implementación de la EPA en una unidad de oncología. Para ello se realizó el proyecto de cambio «Elaboración de un programa de cuidados para pacientes portadores de un catéter drenaje permanente para ascitis y derrame pleural malignos», fundamentándolo en la teoría y en la práctica basada en la evidencia y centrado en el paciente y familia. Su implementación ha permitido el desarrollo de competencias por parte de la EPA y la identificación de barreras y facilitadores. Conclusiones: los cambios surgidos en la sociedad favorecen el desarrollo de nuevos perfiles de enfermería, que tienen un impacto positivo en las instituciones, profesionales de enfermería, pacientes y familias(AU)


Introduction: social and political changes have created an increased in the demand of health care enabling the emergence of diverse professional profiles under the term Advanced Practice Nurse (APN). The APN is a nurse who has acquired a basis of expert knowledge, skills for decision making, and clinical competencies for an extended practice. In Spain, due to the Bologna process and the changes within university, such as postgraduate training, these profiles have begun to emerge. Objective: To describe the experience on the implementation of the advanced clinical nursing role in an inpatient oncology unit. Development: Through the nine stages of the model known as Participatory, Evidence-Based Patient-Focused Process for Advanced Practice Nursing (PEPPA) the process of implementation of the Advanced Practice Nursing (APN) in an Oncology unit is described. For this purpose, the change project "Design of a care program for patients carrying a permanent draining catheter for malign ascitis and pleural effusion" was implemented. The project was grounded on the evidence-based theory and practice, and focused on the patient and the family. Its implementation has enabled the development of competences by the APN and the identification of barriers and facilitators. Conclusions: The changes that have taken place in society favor the development of new nursing profiles, which have a positive impact on the institutions, nursing practitioners, patients and families(AU)

RESUMO

This historical perspective is focused on the contribution of Hildegard E. Peplau in laying the foundation for advanced practice nursing and the development of the roles of clinical nurse specialists and psychiatric nurse practitioners. An overview is provided of legal developments within the state that enabled Advanced Practice Nurses to provide mental health services. A description of a recent specialized state-funded initiative is outlined, focused on the development and contributions of psychiatric advanced practice nurses in community settings in New Jersey. Implications for the advanced practice nursing role in New Jersey are presented based on national and state initiatives.

Advanced practice nursing has evolved over the years to become recognized today as an important and growing trend among healthcare systems worldwide. To understand the development and current status of advanced practice nursing within a Canadian context, it is important to explore its historical roots and influences. The purpose of this paper is to provide a historical overview of the major influences on the development of advanced practice nursing roles that exist in Canada today, those roles being the nurse practitioner and the clinical nurse specialist. Using a scoping review and qualitative interviews, data were summarized according to three distinct time periods related to the development of advanced practice nursing. They are the early beginnings; the first formal wave, between the mid 1960s and mid 1980s; and the second wave, beginning in the late 1980s and continuing to the present. This paper highlights how advanced practice nursing roles have evolved over the years to meet emerging needs within the Canadian healthcare system. A number of influential factors have both facilitated and hindered the development of the roles, despite strong evidence to support their effectiveness. Given the progress over the past few decades, the future of advanced practice nursing within the Canadian healthcare system is promising.
